Job Details
- Hours: Part-time, 15-20 hours per week
- Pay: $24.00 - $26.00 per hour
- Location: Queens territory, hybrid role with some remote work and field visits
- Travel: Must visit clients in the borough, mileage and public transportation reimbursed
Position Overview
- Enroll new members by determining eligibility, explaining services, and obtaining consent
- Develop care plans by assessing needs and creating personalized support plans
- Assist with benefits such as Medicaid recertification, food stamps, and housing applications
- Coordinate care by arranging appointments and ensuring follow-up services
- Meet with clients monthly to provide ongoing support and advocacy
- Maintain records by documenting interactions and progress
Key Responsibilities
- Advocate for members and connect them with the right resources
- Coordinate care between doctors, schools, and community programs
- Provide in-person support through home visits and regular check-ins
- Stay organized with documentation and compliance
- Participate in training to stay updated on best practice
What You Need
- Education: BA with two years of experience or MA with one year of experience
- Skills: Strong communication, organization, and case management skills
- Language: Spanish required
- Other: Must have a valid NYS ID or driver’s license and be willing to travel
