The Clinical Outreach Pharmacist provides clinical pharmacy outreach and support to educate patients, their health care teams, and other stakeholders in order to optimize medication related healthcare outcomes, ensure patient safety, recommend cost-effective medication strategies, address gaps in care measures, support medication adherence initiatives, coordinate care, and assist with strategies to improve HEDIS and STARS quality measures. This position will develop and maintain a program and protocols for Medication Therapy Management (MTM) in collaboration with our Disease Management and Case Management programs. This position will require the Clinical Pharmacist to stay current with literature, evidence-based medicine, and trends in medicine. This position will work with all lines of business with a focus on Medicare Advantage and the Federal Employee Program.

Compensation

112,800 - $134,000 annually

The compensation range for this role is a good faith estimate, it is estimated based on what a successful candidate might be paid. All offers presented to candidates are carefully reviewed to ensure fair, equitable pay by offering competitive wages that align with the individual's skills, education, experience, and training. The range may vary above or below the stated amounts.
  • What you'll do

  • Support initiatives to resolve gaps in care, improve adherence, and perform Medication Therapy Management (MTM) through direct provider and member outreach, including outbound phone calls.
  • Ensure that all recommendations are consistent with the latest guidelines or otherwise supported by the clinical compendia.
  • Collaborate with Case Management and Disease Management teams for member referrals focused on medication safety, education, cost-effectiveness, and other medication related needs.
  • Provide clinical pharmacy support and training to pharmacy technicians and / or pharmacy students as they conduct outreaches to members regarding medication adherence, resolve existing barriers to adherence, and use motivational interviewing to improve the health of our members.
  • Monitor medication utilization patterns using internal systems to improve member outcomes, identify cost-savings for members and the plan, and recommend therapy optimizations.
  • Implement, track, and work on quality improvement projects within the organization.
  • Follow up with patients and providers to check on progress toward meeting drug treatment goals.
  • Serves as a drug information resource to patients, providers and other members of heath care team.
  • Works with other members of health care team (e.g., case managers, disease managers, etc.) on member-centered outreach and interventions.
  • Assists managers and director with developing / updating policies and procedures and implementing changes to comply with state and federal regulations.
  • Collaborate internally with Marketing, IT, and other business areas to assist with the creation of strategic digital and written communication channels for member outreach.
  • Review weekly and monthly reports and provide feedback on trends, ideas for efficiency, and innovation.
  • Develops and maintains reporting to demonstrate value of clinical outreach efforts.
  • Will be an active participant and representative of cross-functional committees and work teams as assigned.
  • What you need

    Knowledge / Skills / Abilities

  • Requires strong knowledge of clinical pharmacy, chronic disease management, biologic and biopharmaceutical products, drug information systems, and drug delivery systems.
  • Requires knowledge of State and Federal pharmacy laws and regulations.
  • Requires knowledge of pharmaceutical care, drug utilization review, utilization management, including URAC accreditation.
  • Requires working knowledge in the evaluation of clinical drug trials, outcomes research and pharmacoeconomics and the ability to apply this knowledge to all areas of responsibility.
  • Requires an understanding of leadership and continuous quality improvement principles.
  • Demonstrates interpersonal, consultative, and problem-solving skills.
  • Exceptional interpersonal listening, and motivational interviewing skills with the ability to interact and work effectively with patients, co-workers, and other healthcare providers.
  • Advanced verbal and written communication skills and the ability to present effectively to small and large groups.
  • Strong organizational skills with the ability to manage multiple tasks and priorities.
  • Excellent patient-focused customer service skills
  • Education and Experience

  • Bachelor of Science in Pharmacy or Pharmacy Doctorate (PharmD).
  • Must have or be able to obtain a current unrestricted license to practice pharmacy by the Kansas State Board of Pharmacy within 90 days of hire.
  • Minimum of three years experience in retail, clinical practice, or one-year post-graduate training in a Post Graduate Year 1 (PGY1) residency with emphasis in managed care, ambulatory care, or MTM.
  • Experience with direct patient counseling and MTM preferred.
  • Board Certification in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or other relevant certification preferred.
