What are the responsibilities and job description for the Transition Care Clinician position at Blue Cross of Idaho?
Blue Cross of Idaho is in search of a clinician to provide timely and quality transitions of care services through comprehensive assessment, planning, and implementation required to fulfill the changing health needs of the member through discharge planning.
This is an onsite role located at our Meridian, Idaho campus and will offer a hybrid work schedule.
What we are looking for in Transition Care Clinician:
Experience: 3 years’ relevant experience
Education: Associate degree in Nursing
Licenses: Valid unrestricted Idaho Registered Nursing License
What your day may look like:
- Maintains communication and collaborates with member, family, physicians, and health team members.
- Plans, implements, and evaluates appropriate health care services in conjunction with the physician treatment plan telephonically.
- Identifies opportunities for intervention.
- Follows the policy and guidelines of the organization.
- Conducts a thorough and objective evaluation of the members’ current status including physical, psychological, and environmental expectations.
- Participates in rounds with the Medical Director(s) to ensure appropriateness to coordination of care services.
- Identifies opportunities for improving efficiencies in the work processes.
- Performs other duties and responsibilities as assigned.

As of the date of this posting, a good faith estimate of the current pay range is $71,708 to $100,391. The position is eligible for an annual incentive bonus (variable depending on company and employee performance.
The pay range for this position takes into account a wide range of factors including, but not limited to, specific competencies, relevant education, qualifications, certifications, relevant experience, skills, seniority, performance, shift, travel requirements, internal equity, geography, business or organizational needs, and alignment with market data. At Blue Cross of Idaho, it is not typical for an individual to be hired at or near the top range for the position. Compensation decisions are dependent on factors and circumstances at the time of offer.
