This full-time role requires someone with a strong healthcare background and sales experience, ideally with a track record of generating referrals and increasing admissions. CARE ACCESS CONSULTANT - - POSITION SUMMARY
The Care Access Consultant is responsible for developing, maintaining , and growing a network of referral sources within the Blue Ridge Hospice service area. The Care Access Consultant articulates the Blue Ridge philosophy, mission, and vision while modeling ethical behavior, demonstrating a positive attitude, acts with integrity, performs with excellence, and demonstrates commitment to the organization. The position is responsible for increasing referral and admission volume to meet established admission goals. The successful Care Access Consultant will have the ability to navigate the healthcare environment and a track record of referral generation and conversion. The liaison will also serve as a speaker in outreach activities to educate healthcare referral sources, community professionals, influencers and consumers about Blue Ridge Hospice services.

CARE ACCESS CONSULTANT - - ESSENTIAL DUTIES & RESPONSIBILITIES
Profile s, analyzes, and prioritize s referral sources within assigned territory by making phone calls, conducting presentations, and in person visits.Actively participates in marketing meetings and implements and executes marketin g strategies.Works with the clinical and operational teams to promote the highest level clinical experience.Creates individual, quarterly / annual marketing plan to promote professional and community awareness and build referring relationships with healthcare providers , local and area hospitals , and health systems.Serves as a liaison with team members , p roviders , vendors, hospitals, community-based facilities, and individuals t o identify their needs to educate and grow the use of Blue Ridge Hospice and Palliative Care services in their assigned territory as demonstrated by earlier referrals and / or increased referrals.Asks the referral source for a commitment or action to increase referrals to Blue Ridge during every visit.Meets the established goals for visits, contacts, number of admissions, and other goals set by manager .Maintains and updates the CRM system to track interactions, leads, and follow-ups.Demonstrates the ability to track and analyze referral patterns and outcomes.Organize s and utilize s hours during each day to maximize time with referral sources a nd available as needed for events after core hours and weekends as needed.Works in conjunction with clinical management to improve communication with referral sources.Understands Medicare, Medicaid and Private Insurance and all other payor sources for Blue Ridge services.Comprehends and demonstrates the ability to communicate and educate on criteria for hospice and other services offered by Blue Ridge.Gathers and analyzes feedback from referral sources to improve service delivery and relationship management.Meets with caregivers and / or family members to share information regarding hospice services, lead family meetings, and obtain admission consents.Serves as an active member of the hospice team and attends team meetings.Provide s education for referral sources and works with Director of Growth & Access, specifically for support for clinically focused topics.Completes all job duties in a manner that meets accepted standards of practice and Blue Ridge Hospice Policies and Procedures and attends all mandatory meetings.Acts independently and responsibly to perform job duties .Understands both state and federal guidelines / laws for hospice services including Stark laws and its application in marketing to referral sources.Respects the autonomy and right to confidentiality of all patients and caregivers .Ensures effective working relationships are maintained with healthcare facilities to enhance utilization of Blue Ridge Hospice services.Ensures a positive working relationship and prompt resolution of identifying problems with physicians , referral sources and team members.Coordinates and participates in community events to enhance the presence of Blue Ridge Hospice Services in both medical and in the general community as approved by the Chief Growth and Access Officer.Conducts face to face discussions with community and medical referral sources to identify need s related to end of life care.Represents Blue Ridge Hospice as a speaker at community events and other public forums.Perform s an initial , quarterly, and annual market analysis and plan.Demonstrates the ability to analyze both internal and external data to develop a marketing plan.Completes expected assignments in a timely manner .Utilizes the patient software program proficiently.Possesses interpersonal skills that promote effective teamwork and communication.Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
