POSITION SUMMARY
We are seeking a compassionate and skilled Hospice Music Therapist to join our dedicated interdisciplinary team. In this role, you will use music to provide emotional, psychological, and spiritual support to patients and their families during end-of-life care. Through personalized music therapy interventions, you will help alleviate symptoms of pain, anxiety, and depression, while enhancing quality of life and fostering moments of connection, peace, and comfort. The Hospice Music Therapist will collaborate closely with the hospice care team, contributing to individualized care plans and providing therapeutic support in patients' homes, hospice facilities, or other care settings. This role requires a deep commitment to compassionate care in a sensitive and rewarding environment.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide direct service to hospice patients and their families, in homes, inpatient units and alternative homes.
Consult with referring team and other interdisciplinary team members about patient referrals to assess needs and to prioritize intervention.
Participate in care conferences as appropriate.
Conduct assessments and provide interventions that meet individual needs.
Promptly complete and maintain all required documentation.
Educate hospice staff about the role of the Music Therapist and the use of music therapy interventions with hospice patients / families.
Continual program development and evaluation of music therapy services.
Assess music therapy equipment including inventory, maintenance, and use of equipment.
Assist with facilitating support groups upon request.
Provide bereavement follow-up to family members when determined appropriate by team members involved.
Assist with orientation of new employees and volunteers.
Assist with memorial service upon request both internally and externally to be an active participant in the planning and implementation of Camp Hope.
Be available for emergency situations.
Follow appropriate infection control policies.
Conduct in-service programs and assist with outside public relations activities as requested.
Participate in agency committees as requested.
Respects the autonomy and right to confidentiality of all patients and families. Recognizes limitations on rights to confidentiality and self-determination in situations of mandatory reporting of abuse, neglect, and intent to do harm.
Is sensitive to the stressors experienced by colleagues in working in the home care environment.
Fosters the human dignity and personal worth of each patient.
Participates in the development of policies, procedures, clinical protocols, form, and service programs as requested. Participates in departmental and / or agency-wide quality improvement activities.
Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback. Seeks assistance of Team Leader or other resource persons in responding to opportunities for improvement.
Acts independently and responsibly to perform job duties on a consistent basis.
Carries out the mission and values of Blue Ridge Hospice.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and in good working order. Must be willing to travel the entire service area.
EDUCATION AND EXPERIENCE
Bachelor’s Degree required.
Registered and Board Certified by AMTA.
Experience with terminally ill preferred.
