Blue Sky counseling is seeking an experienced Case Manager to fill a growing need in the Skowhegan/Waterville and surrounding areas. As the need has grown for the people Blue Sky serves, we have decided to implement community integration services to complement our substance use treatment services that have been imbedded in this community for years. We welcome applicants that have been versed in community-based services, are self-starters, motivated and have the drive and ambition to help us grow this program. This is a new program for us and we are open to part-time or full-time hours based on employee preference with the ability to increase hours.

Job Title: Community Integration Specialist- Case Manager

Job Purpose: Community Integration Specialists are responsible for providing resources and helping clients connect to services to reduce mental health symptoms and increase independence. Community Integration Specialists assist by assessing all treatment needs, develop individualized treatment planning based on identified needs and implement measurable tasks to meet those needs. Community Integration Specialists also monitor progress, problem solve as needed, provide symptom management and empower clients through the process. The ability to work independently as well as a member of a team, maintain professionalism and ethical standards, and possess strong written and verbal communication skills are all necessary in this role.

Essential Responsibilities:

  • Provide support, advocacy, and oversight to individuals served according to their personalized treatment plans.

· Provide ethical services by maintaining professional relationships with the client and by working within the role of case manager.

· Ensure services are provided to clients in a manner consistent with trauma-informed practices, emphasizing strengths, privacy, individuality, choice and cultural differences.

· Provide services to empower the client and family system to utilize and build their strengths, resources and support to meet goals as independently as possible.

  • Assesses, evaluates and develops a plan of care consistent with the needs of the individual.
  • Establishes links within the community to include making referrals, coordination, and advocacy to facilitate access by the individual to the services and support identified by the individual within their assessment and Individualized Service Plan (ISP).
  • Develops strong and effective working relationships with external and internal stakeholders, including, but not limited to public/private guardians, provider agencies and community vendors.

· Fully participate in required supervisions and trainings by coming prepared, actively engaging and documenting as required.

· Maintain and complete accurate client files according to agency policies, procedures and in compliance with applicable law, licensing, contracting, Maine Care regulations, consent decree and Rights of Recipients.

· Independently and effectively manage caseload, including scheduling of client appointments, meetings and office time for completion of required paperwork.

Paperwork Requirements:

· All intake, consent and annual documents needed to provide treatment.

· Completing thorough comprehensive assessments.

· Developing individualized service plans, including a crisis plan.

· Be able to support with referral forms, applications and other documentation for outside services as needed.

· Document all client contacts and all collateral coordination of services contacts.

· Ongoing monitoring to assess safety, well-being, and stability, as well as monitoring the progress toward the goals identified in their individual care plan.

Qualifications:

  • High School Diploma or GED is required, related associate degree preferred,
  • Valid State issued Drivers’ License and proof of current insurance required.
  • Must have MHRT-C or MHRT-C Provisional and maintain with 18 approved CEU’s every 2 years
  • Minimum of 2 years of related experience in the field working with clients in the community

· Strong English written and verbal communication skills are required

· Working knowledge of Microsoft Office, Microsoft Outlook, Zoom and other video conferencing software

· Ability to work independently and as a part of a team.

Coverage Area:

Community Integration Specialists will work in the community and within client’s homes in Kennebec and Somerset counties. The position will be based out of Skowhegan with a minimum of once-a-week travel to the Waterville office and an estimated 25-mile radius of Skowhegan.

An office will be provided in Skowhegan for paperwork, office-based work, trainings and supervision, space will also be available as needed in the Waterville office for days designated to see client’s closer to that area.

Job Types: Part-time, Full-time

Pay: Starting pay for this position is $22.00 an hour, pay will be based on experience. Bonuses are also available based on hours worked and percentage of billable hours per week. Travel stipend included.

Schedule:

  • This position will be Monday to Friday 8 a.m.-5 p.m. for full time and set weekly daytime hours for the right part time employee.

*Competitive pay based on experience bonus scale based on billable hours.

MHRT-C required for this position
