What are the responsibilities and job description for the Community Developer II/Case Manager position at Blue Sky Innovative Solutions?
Blue Sky Innovative Solutions is looking for 2 temporary Community Developer II/Case Managers. Provide information regarding our Customer's Government Program. Respond to inquiries regarding eligibility. Conduct quarterly assessments and complete excel spreadsheet with assessment information. Enroll clients into county Program and update files.
LOCATION
A1. Provides assistance to the Adult Public Guardianship staff to assure that program goals/objectives are achieved.
A2. Participates in the Department of Social Services APS Inter-Agency Team and provide updates to the Unit Manager.
A5. Assist the Guardianship Specialist in preparing reports. Reports are prepared accurate and in a timely manner.
B1. Provides office coverage during normal business hours as needed.
B2. Communicates with caregivers and health care workers regarding updates pertaining to Guardianship Wards.
B3. Conducts visits bi-monthly at nursing homes, assisted living facilities and private residences to ensure that all wards are receiving appropriate care.
B4. Updates case files with documentation of in-person visits and telephone contacts.
C1. Prepares request for medical information pertaining to clients from nursing homes, assisted living facilities, and medical staff.
C2. Participates in scheduled care plan meetings and provides updates to the Unit Manager.

Minimum Qualifications
- Must have at least 2 years of community developer working experience
- Have experience in planning and coordinating services, handling case assignments drafting service plans, reviewing case progress, making referrals.
- Have experience in providing crisis intervention.
- Have experience in addressing parent/child conflict, teaching pro-social skills, and improving relationships between the family, community, and school.
- Ability to operate a motor vehicle.
- Must have a valid driver's license.
- Strength and dexterity necessary to perform all required tasks including lifting, stooping, bending, and working in tiring and uncomfortable positions.

- High School Diploma or GED
- Must have the use of sensory skills to effectively communicate and interact with other employees and the public using the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. No significant climbing, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, grasping, and repetitive motions.
