JOB SUMMARY:
To deliver varied social work services to Hospice patients and their families. To provide initial
emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services and
community education, outreach and referral. The Hospice social worker is an integral part of the
Hospice IDG.
