Are you passionate about supporting individuals with severe and persistent mental health challenges on their journey to recovery?

We’re looking for a skilled and compassionate professional to join our dynamic interdisciplinary team in Guadalupe County. In this role, you’ll provide high-quality, recovery-oriented case management and skills training services, making a meaningful difference in the lives of those we serve.

Why You’ll Love This Role:

  • Collaborative, Supportive Environment: Work alongside a diverse team of nurses, peers, psychiatrists, and other professionals, all dedicated to delivering integrated care that’s both innovative and effective.
  • Empowerment through Skills Training: Use your expertise to teach individuals essential skills for independent living, community engagement, and overall wellness. Your work will directly promote community integration, increased stability, and enhanced quality of life for individuals navigating mental health challenges.
  • Impactful Work with a Purpose: Your efforts will help individuals reintegrate into the community post-hospitalization, focus on safety planning, and work on building the strengths and natural supports needed to thrive. Every day, you’ll make a positive difference in the lives of those most in need.

What You’ll Be Doing:

  • Recovery-Focused Case Management: Provide person-centered case management services aimed at reducing barriers and enhancing wellness, independence, and stability.
  • Skills Training and Psycho-Social Support: Facilitate skills training to help clients overcome challenges and build the tools needed for a successful recovery journey. This may include face-to-face or televideo sessions in-clinic or in the community.
  • Collaboration with Mobile Crisis Outreach Team (MCOT): Work closely with MCOT to ensure seamless continuity of care, from crisis management to recovery support. You’ll also conduct crisis assessments to evaluate risks and facilitate appropriate referrals for further treatment.
  • Building Resilience and Safety: Support individuals with high-crisis needs (including those with recent MCOT contacts, crisis line calls, or psychiatric hospitalizations) to build resilience, reduce risks, and develop a sense of empowerment in their recovery process.

Why This Role Is for You:

  • You have a deep commitment to mental health recovery and empowering individuals to live fulfilling, independent lives.
  • You thrive in a collaborative team environment and enjoy working with other professionals to ensure the best outcomes for those you serve.
  • You bring strong crisis management and assessment skills and are motivated by the opportunity to help individuals overcome complex challenges in their recovery journey.
