Location: Lexington, KY
Schedule: Monday-Friday 8am-5pm, with occasional on-call and weeknight afterhours call

We Offer
The Palliative Care Nurse Practitioner exemplifies and promotes advanced nursing practice and excellence in Palliative Care, and provides comprehensive, quality care to individuals living with serious illness. A Nurse Practitioner in this position will focus on providing relief from the symptoms and stresses of a serious illness with the goal of improving the quality of life for patients, caregivers, and families. The Palliative Care Nurse Practitioner will be responsible for providing visits for both new and established patients, which may include visits to patients’ homes, facilities, and hospitals. This position works in close collaboration with both community and Bluegrass Care Navigators’ physicians, as well as our community partners through established palliative symptom management protocols.
The Nurse Practitioner plays an integral role in formulating a plan of care for the patient, family, and/or caregiver and assists with facilitating clear communication of this plan to Bluegrass Care Navigators’ physicians, the patient’s primary care provider, or specialist as appropriate. The Palliative Care Nurse Practitioner may help develop practice protocols for palliative and end-of-life care or educate other clinicians about the palliative care specialty while maintaining one’s own professional knowledge and skills as an expert and role model in Bluegrass Care Navigators’ advanced nursing practice.

Requirements
- Is a graduate of a Master’s Degree in a Nursing Program. APRN certification as Family or Adult Nurse Practitioner. In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice.
- Is currently licensed as an APRN through the State Board of Nursing.
- At least three (3) years of general nursing experience is required with current hospice, palliative care, home health, medical, surgical or critical care experience. Has experience with providing good pain management. I.V. skills preferred.
- Skilled in nursing practice, able to cope with family emotional stress, and tolerant of individual lifestyles.
- Committed to the research process and application to practice.
- Ability to organize resources appropriately.
- Strong interpersonal skills, communication, collaboration and conflict resolution.
- Knowledge of systems and the change process.
- Ability to manage multiple priorities simultaneously and effectively handles the emotional stress of the workload.
- Complies with accepted professional standards and principles.
- Possesses and maintains current CPR certification.
- Have satisfactory references from nursing school, previous (or current) employers, and/or professional peers.
- Has excellent observation, good nursing/medical judgment and communication skills. Ability to provide good written documentation in a timely manner.
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily coverage
