We are seeking a Case Manager to serve our foster children and foster families in our Barbourville, KY service area!
Hours are flexible to best meet the needs of children and families served. Minimal weekend and evening hours may be required to ensure children receive the best care with a rotating on-call schedule.
Office located on the campus of Appalachian Children’s Home in Barbourville, KY. This position may serve foster families in any of our Barbourville office service counties including, but not limited to, Knox, Bell, Clay, Laurel, Whitley, and Jackson
SUMMARY OF POSITION RESPONSIBILITIES:
Under the supervision of the Director of Case Management, the Case Manager is responsible for planning and coordinating services for clients of the Bluegrass Foster Services. This position is crucial in the management of client services, care, and the organization of client records. Their caseload shall not exceed twelve (12) therapeutic leveled children or twenty (20) non- leveled children.
MINIMUM QUALIFICATIONS:
- A bachelors degree in the human services field.
- Ability to apply the concepts of customer service in the workplace. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.
- Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills.
- Ability to handle multiple tasks and meet critical deadlines. Ability to meet short and long-term goals and project deadlines.
- Respect for the confidentiality of all communications required within job duties.
- Pass criminal background check and drug screen upon hire and at random during employment.
- Maintain a valid driver's license, along with current vehicle registration and insurance, as local travel is required.
ESSENTIAL DUTIES include, but not limited to:
- Complete the intake process on children admitted to Bluegrass Foster Services.
- Meet face-to-face with each youth at least twice per month; at least one in home visit and one out of home visit.
- Have contact with each foster parent at least once a week.
- Plan, schedule, and conduct quarterly treatment team meetings for each child.
- Maintain an electronic health record for each child with all required documentation.
- Collaborate with the child's DCBS social service worker in regards to all matters pertaining to the child's treatment.
- Participate in the company on-call rotation to ensure a BFS staff is available to children and foster families in crisis situations 24/7.
- Participate in marketing and activities related to growth and development of the agency.
- Other duties as assigned.
