Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. The Case Manager will work collaboratively with various stakeholders to ensure clients receive the necessary support for their personal and professional development.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop individualized service plans.
- Provide ongoing support and counseling to clients, utilizing techniques such as motivational interviewing.
- Facilitate access to resources including workforce development programs, addiction counseling services, and housing assistance (e.g., Section 8).
- Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
- Collaborate with community organizations, healthcare providers, and other stakeholders to coordinate services.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Engage in crisis management strategies to address urgent client needs effectively.
- Advocate for clients’ rights and access to services within the community.
Experience
- A degree in social work or a related field is preferred; relevant experience may substitute for formal education.
- Proven experience in case management or a similar role within behavioral health or social services.
- Familiarity with workforce development initiatives and addiction counseling practices is highly desirable.
- Strong crisis management skills with the ability to remain calm under pressure.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
- Knowledge of Section 8 housing programs and related social services is an asset.
- Experience utilizing motivational interviewing techniques to empower clients toward positive change.
