Position Overview:
Under the direction of the Clinical Service Director, this individual is responsible for rendering skilled nursing services, palliative care services as well as planning and monitoring of in-home services. Assist Clinical Service Director in the supervision of the home care aides employed by Bluestem PACE.
Essential Job Functions:
- Responsible for the skilled nursing care, palliative care and home care needs of the group of participants assigned.
- Responsible for the completion of care plans utilizing the nursing process of assessment, establishing goals, planning interventions and evaluations for the assigned participants.
- Provide comfort/palliative care at end of life.
- Responsible for a documented encounter at least monthly on assigned participants.
- Responsible for the completion of the supervisory visits on the participants assigned and notifying Clinical Service Director if care issues arise.
- Participate in quality assurance activities as assigned.
- Assure that documentation for home care is accurate, current, and that the home care charts are up to date.
- Provide accurate and timely utilization and PACE data.
- Provide off-hour on-call assistance as assigned for the completion of skilled nursing and palliative care visits occurring at that time.
- Provide direct patient care in home and at clinic based upon policy and procedures standards of care and regulatory bodies.
- Prepare and administer medications, enteral nutrition, parenteral nutrition and treatment as ordered.
- Attend and participate in staff meetings, in-services, projects and committees as assigned.
- Adhere to and supports the facilities’ policies, practices and procedures.
- Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
- Must be flexible in schedule of hours worked.
- Does require use of personal vehicle.
Qualifications and Experience:
- Must be at least eighteen (18) years of age
- Kansas Registered Nurse degree from an accredited school of nursing or college/university
- Must have working knowledge of Microsoft Office Suite
Physical Requirements:
Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
