Mercy Health seeks people that are committed to our values of compassion, human dignity, integrity, service and stewardship to create an environment where associates want to work and help communities thrive.Psychoeducation Specialist – St. Charles HospitalJob Summary : The Psychoeducation Specialist provides direct patient care under the supervision of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health (BSMH).Essential Functions : Facilitates individual and / or support groups (within Psychoeducation scope) and may participate in family meetings and treatment team meetings.Provides community resource knowledge and handles coordination of related activities.May provide 1 : 1 patient safety companion service as needed, and patient checks as assigned.Care planning and discharge planning in collaboration with Social Worker team.Participates in continued education and in-service training.This document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job.
