Description
Access clients entering into the treatment program by completing all the required paperwork which will include the following: intake paperwork and bio-psychosocial assessment and GPRAs. If client is deemed not appropriate for the treatment program, therapist will help client look at alternative options.
Conduct individual therapy sessions as required
Conduct group therapy sessions as required
Complete and revise patient treatment plans as required per state regulation.
Complete discharge summaries within a 24-hour period once a client is no longer part of the treatment program.
Attend weekly staff meetings
Keep a record of all CEUs completed to be able to get or keep licensure up to date.
Monitor compliance to legal guidelines, internal policies and quality standards

- Help assess patient interest in and appropriateness for the MAT program.
- Assists with managing MAT patient registries to promote patient care plan adherence.
- Work in cooperation with the MAT providers and other staff to assist patients through the healthcare system by acting as a patient advocate.
- Facilitate patient education and access to healthcare and community resources. Facilitate continuity of patient care with external healthcare organizations and facilities, including routine communication with SUD treatment centers, helping patients gain admission into treatment centers, etc.
- Document needed patient information in an accurate, concise, and timely manner.
- Demonstrate competence in information management in both written and electronic documentation systems.
- Actively participate in staff meetings and trainings, and other meetings as appropriate.
- Participate with other community agencies to share knowledge that will improve quality of care for the MAT patient population.
- Promote and model collaborative relationships with all partner agencies.
- Recognize the role that the cultural diversity of patients and their families, employees, medical staff, volunteers and community members plays in achieving productive and positive relationships.
- Uphold and support Brighter Futures mission statement.
- Help assure compliance with Federal, State and program guidelines and requirements pertaining to the care of patients with opioid use disorder.
- Safeguard all forms (electronic, written and oral) of confidential information as it relates to patients, their families, medical staff and employees.
Competencies
- Completing required yearly CEUs

Physical demands
Ability to perform various motor skills such as; standing or sitting for long periods of time, waking, lifting (up to 25 pounds) using proper lifting techniques, stair climbing, reaching, bending, twisting, pushing, pulling, squatting, grasping, and arm-hand coordination on a continuous basis.

Required education and experience
Masters degree in Social Work or Counseling or other social services.
Preferred education and experience
LPC, MSW, LGSW, LICSW and one year of experience dealing with substance abuse patients preferred.
