In this leadership role, you’ll collaborate with the Office Administrator to develop and guide the clinical team, ensuring high-quality care coordination and follow-up. You will also oversee adherence to each patient’s Plan of Care (POC). Experience with OASIS, PDGM coding, and 485 / Plan of Care is required.
Candidates must have at least one year of home health experience and two years of direct patient care , ensuring a solid foundation in patient-focused, quality-driven service.
In addition, the ideal candidate will possess knowledge of OASIS, PDGM, QAPI functions, Medicare Conditions of Participation, and state regulations. Strong computer skills, meticulous attention to detail, and excellent verbal and written communication are also essential for success in this role.
Responsibilities :
Oversee and coordinate clinical operations across Frisco, Plano, and Hebron to ensure efficient workflows and exceptional patient care. (Primary office location : Frisco)Evaluate and re-evaluate patient conditions, update care plans, and address patient needs per company, physician, and facility policies.Assign and manage caregiver caseloads effectively, ensuring the right level of care.Review incoming referrals, confirm eligibility, and recommend necessary services.Select and guide clinicians for each case, providing real-time support and instruction.Collaborate with clinicians to establish therapeutic goals, prioritize patient needs, and develop comprehensive Plans of Care (POCs).Ensure OASIS documentation is accurate, timely, and compliant with federal and state regulations, offering support and guidance to clinicians.Monitor patient cases to confirm documentation meets all regulatory and third-party payer requirements; complete audits and billing in compliance with Medicare regulations.Supervise team members in line with company policies, contributing to interviewing, hiring, and ongoing staff development.Evaluate clinician performance, provide coaching, and recommend in-service training as necessary for continuous improvement.Lead QA / PI activities to maintain and elevate clinical standards.Manage patient care assignments and oversee staff performance, stepping in to provide direct patient care only in emergencies.Serve as Branch Director in their absence, maintaining leadership continuity.Interpret and enforce Boost HHC policies to ensure regulatory compliance and accurate clinical documentation.Engage in performance improvement initiatives and pursue ongoing clinical education through internal and external training programs.Perform other related duties as required or assigned to support overall clinical operations.What we’re looking for :

Salary : $90,000 - $110,000
