Seeking a Family Nurse Practitioner with an Oncology background to work in BCHP's Adult Infusion Center in Hawthorne, NY. Candidates must be board certified as a Nurse Practitioner. Salary will be $100k-$120k negotiable based on experience. Schedule Monday-Friday approx. 9:00am-5:00pm.
Essential Tasks, Duties and Responsibilities
- Provides teaching and counseling in the areas of health promotion, maintenance, disease prevention and management
- Review, evaluate and make recommendations regarding laboratory findings for patients seen in the infusion center
- Manage post infusion follow-up
- Manage infusion reactions and treat as appropriate
- Work collaboratively with the oncology team to ensure all accreditation guidelines are met
- Evaluate, diagnose, and treat patients as appropriate according to the scope of practice
- Use evidence based, shared decision making tools whenever possible
- Document all encounters in the EMR
- Bill for services appropriately
- Perform population level health assessments such as vaccination reports to ensure that patients do not slip through the cracks and miss vaccinations or other tests
- Support the nurse and medical assistant on the care team
- Provide care in a patient-centered manner
- Attend medical home meetings
- Allow care team to work under standing orders to the top of their license
- Participate in pre-visit planning
- Maintain board certification and enrollment in professional societies
- Demonstrate excellence in both internal and external customer service
- Other duties as assigned
Organizational Duties and Responsibilities
- Performs duties to support the goals and objectives of Boston Children's Health Physicians in its efforts to provide quality service and patient satisfaction
- Demonstrate continuing education by completing all required annual trainings, as stated in the Training Curriculum, including but not limited to care coordination, population health management and self-management support techniques.
- Complete annual corporate compliance training
- Understand and effectively communicate HIPAA compliance, corporate compliance and client confidentiality
- Ensure and/or remain in compliance with local, state and federal regulation
