AZ-Clinic Care Coordinator-RN-DAYS

Primary Care Clinic at Tsehootsooi Medical Center.Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:Requirement: Three (3) year Case Management Experience.Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.Housing: Information can be found under the 'Facility Group Document section. Orientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.

Shift: M-F, 8-5/730-430, no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contract.
