Characteristics of Work:
This is a semi-skilled level of work in the care and supervision of residents or individuals in a community-based setting. The Contract Worker / Job Coach will independently access the community and maintain liaisons with community and organizations; develop employment skills of individuals served and provide information and direction to individuals to ensure job assignments are completed per requirements of employers. Diligently monitors and reports on the physical and mental health of the individuals served; establishes positive relationships by quality communication, appropriate interactions, and the ability to create a balanced relationship with clear boundaries. Will be required to complete progress notes and any other pertinent paperwork for each shift worked. Attends all in-service training required for the position monthly, yearly, or as needed, also performs related duties as required or assigned.
Examples of work / expectations:
· Communicate clearly in oral and written forms to provide necessary support and produce a variety of documentation which meets or exceeds state and agency standards.
· Responsible for providing the personal assistance and supervision required for individuals served to improve and maintain quality of life as outlined in Individual Service Plans (ISPs).
· Assure that individuals served are completing job assignments.
· Builds and maintains relationships with business partners when providing on-the-job support for individuals working in the community.
· Responsible for diligent monitoring and reporting of the physical and mental health of individuals served including any health changes and any out of the ordinary occurrences.
· Responsible for writing progress notes for each shift worked that meet or exceed agency and state standards and meet established timelines.
· Responsible for completing all applicable documentation according to agency standards and timelines connected to service for all shifts worked.
· Follow individual Service Plans to ensure excellent service provision.
· Shares pertinent information with necessary parties in a timely manner.
Essential Functions:
· Ability to oversee individuals served are completing assignments per the requirements of business partners.
· Ability to problem solve independently while providing support to the individual at the job site.
· Keeps supervisor informed of important incidents, plans, issues concerning individuals served.
· Strictly adheres to confidentiality guidelines.
· Must meet the requirements for driving a state vehicle which are:
1) valid driver’s license,
2) possession of liability insurance on personal automobile
3) possession of a good driving record.
Minimum Qualifications:
· Must have a high school graduate or equivalent (GED) and be at least twenty-one (21) years of age.
Any and / or all requirements are listed on the State Personnel Board Class Specifications and are applicable to the requirements of this position.
