What are the responsibilities and job description for the Support Care Professional I, II, III position at Boswell Regional Center?

Third-level Support Care position accountable for taking vital signs, assisting with care plan activities, observing, and reporting patient conditions and behaviors, and participating in rehabilitation, ambulation, and transfers. May mentor or direct the work of lower-level employees. Communicates with doctors, patients, and families, and educates patients on care. May provide transport to offsite appointments and application of restraints when necessary.
Examples of Work:
· Accountable for assisting with care plan activities.
· Responsible for developing and/or implementing habilitation or treatment plans.
· Accountable for teaching or assisting in developing client skills related to self-help, play, education, and socialization.
· Responsible for observing and reporting patient conditions and behaviors.
· Accountable for participating in rehabilitation, ambulation, and transfers.
· Accountable for participating in the planning and carrying out of classroom instruction activities.
· Responsible for carrying out training programs as specified in the client ISPs.
· Responsible for monitoring client interactions while they are in the Day Services areas.
· Accountable for insuring client safety while at Day Services.
Minimum Qualifications:
Typically requires High School Diploma or high school equivalency, and 3-5 years of experience. May require a Certified Nurse Assistant (CNA) designation.
