What are the responsibilities and job description for the LPN/RN; Wound Clinic; PRN position at Bothwell Regional Health Center?
The Wound Care LPN (Licensed Practical / Vocational Nurse) is responsible for all assigned nursing care activities to include but not limited to wound care for patients of all ages and other clinical activities associated with providing safe patient care. The LPN will be responsible for assisting with patient and wound care data collection for outcome based care management.
The LPN must be able to demonstrate the knowledge and skills necessary to provide care to patients served on his/her assigned unit as described in unit-based competencies, policies, and procedures.
In collaboration with other staff members, the LPN provides bedside patient care, implementing the interventions identified in the plan of care, as directed by the RN.
The Clinic LPN reports directly to the Director of Clinic Operations.
QUALIFICATIONS:
Education:
Graduate of an accredited Practical School of Nursing.
Licensure/Certification:
Current licensure by Missouri State Board of Nursing.
BLS certification required within 6 months of employment.
Work Experience:
Successful completion of orientation for LPN.
RESPONSIBILITIES/JOB DUTIES:
Ensure appropriate patient care documentation.
Implement and assure compliance with wound center and hospital policies and procedures.
Maintain familiarity with regulatory standards.
Report quality of care issues to staff RN or Program Director.
Ensure availability of necessary equipment and supplies.
Communicates with all team members regarding therapeutic interventions.
Perform other duties as assigned.

and experience.

Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care.
Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled, and is dependable. Able to manage a number of patients at the same time with high energy and organization.
Collaborates with other health care providers, wound care center Physicians, Program Director, and Medical Director regarding clinic and patient needs.
