What are the responsibilities and job description for the Licensed Clinical Social Worker position at Boulder City Hospital?
GENERAL PURPOSE
The Social Worker is responsible for assisting residents and families with social and emotional issues, including implementation of social services care plan.
ESSENTIAL FUNCTIONS :
Ability to lift, walk, carry and stand. Demonstrates skills and abilities to collaboratively work with all disciplines; must be familiar with Federal and State Long Term Care regulations.
DUTIES AND RESPONSIBILITIES
1. Assists with facility marketing through outreach and public speaking engagements.2. Obtains Psychosocial history and assessment, input on all MDS assessments, care plans / progress notes, initially and quarterly on all Long Term residents.3. Maintains accurate and timely documentation on all patients and referrals.4. Works with third party providers to facilitate transfers, discharges and associated reimbursement or paperwork requirements.5. Participates in resident care plan sessions, meets with Nursing Supervisor to review patients weekly and consult with nursing staff daily.6. Provides inservice training for facility staff on socio-emotional topics, and participates in Hospital inservice programs on an ongoing basis.7. Is responsible for job specific training of departmental employees.8. Maintains confidentiality of Medical Records and information divulged by patients and families, except for information given which can be physically harmful to ones self or others will be reported.9. Acts as resident advocate. Acts as liaison between Boulder City Hospital and the community as an educational resource.10. Provides information and referrals to hospital personnel, as well as the community for community resources.11. Reviews and reports all possible child or elder neglect and / or abuse to appropriate agencies such as child protective services or senior protective services.12. Responds to drug and / or alcohol referrals for appropriate referrals for assistance.13. Responds to referrals for patients with psychiatric problems.14. Provides crisis intervention to patients and / or family members when deemed appropriate, or when referral has been given to the social worker to assist with crisis.
SKILLS AND ABILITIES REQUIRED
1. Excellent assessment, listening, counseling and communication skills.2. Ability to communicate effectively.3. Occasional local travel.4. Experience in a healthcare environment.5. Ability to perform work tasks within the physical demand.6. Genuine caring for and have an interest in elderly and handicapped people.7. Ability to comply with the Patient Bill of Rights and the employee responsibilities.8. Ability to comply with Boulder City Hospital and departmental safety policies and procedures.9. Ability to read and write English.10. Ability to pass high efficiency respirator it and use program as required by OSHA, Volume 58, No. 195.11. Ability to perform group sessions with residents as it correlates with Activity Director.
EDUCATION AND EXPERIENCE REQUIRED
