Boulder County Public Health Department is seeking to hire a Bilingual Children with Special Needs (CSN) Program Nurse.  This position performs professional generalist nursing functions to support optimal growth and development for all children, with a particular emphasis on children who have special health care needs. The position provides nursing consultation services to families of children with special health care needs with an emphasis on neonatal intensive care unit (NICU) newborn graduates residing in Boulder County. The position works within the community to strengthen systems of care that support the medical home concept of easily accessible, comprehensive, coordinated care for all children and their families. This position will also work within the Assuring Better Child Health and Development (ABCD) Committee in Boulder County to address priorities and activities related to development screening, referral, evaluation, and services for children ages birth to three years. This position requires extensive travel throughout Boulder County. 

This is a grant funded position by the Boulder County Individuals with Intellectual or Developmental Disability (IDD) Mill Levy with a good chance of renewal. Current term expires on June 30, 2025, however there is a strong possibility of continuation funding for this position.  This position will work Monday-Thursday 7:00 am - 5:00 pm or Monday- Friday 8:00 am- 5:00 pm.  This position will work at the St. Vrain Hub located at 515 Coffman St. Longmont, CO.  Under Fair Labor Standards Act (FLSA) guidelines, this position is exempt.


Boulder County requires its employees to reside in the state of Colorado as of the first day of work. 

 

Hiring Salary Range: $72,360.00-$88,284.00 Annually

 

Tentative Hiring Timeline:

  • Language Proficiency Test: February 18th
  • Reference Check: February 18th

  1. Conducts the physical, psychosocial, and developmental assessment of the child and family clients to determine their needs and goals; develops comprehensive, individualized health care plans for them.
  2. Collaborates with the family and interagency multi-disciplinary team to provide various levels of care coordination, including information and referral, health monitoring for "at-risk" populations, and nursing consultation.
  3. Represents BCPH and the public health nursing discipline on interagency and interdisciplinary teams.
  4. Provides prevention and early intervention on issues related to children and youth with special health care needs.
  5. Participates in the monitoring of the children and youth with special health care needs population through data collected from newborn screening programs, neonatal intensive care unit hospital referrals, and the Ages and Stages Questionnaire (ASQ) developmental monitoring program, in addition to other population­based data sources.
  6. Promotes public health concepts in health system development within the community.
  7. Works with children and youth with special health care needs from culturally diverse families, both in the home and within the community.
  8. Assists parents/caregivers in establishing realistic goals for promoting optimal child/youth health and development.
  9. Develops an effective working relationship with families to enhance problem-solving competence.
  10. Coordinates and collaborates closely with community resources to link children and families with appropriate services and improve outcomes.
  11. Documents nursing activities in a timely manner and in accordance with CSN Program protocols.
  12. Maintains accurate and complete client files and records, and contribute to grant reporting.
  13. Performs related duties, as assigned or required.

EDUCATION & EXPERIENCE:

Boulder County is looking for well qualified candidates to fill our positions. Any combination of relevant education and experience is encouraged. In this position, we are looking for a minimum of an associate's degree in nursing and four years of professional nursing experience.

Additional related experience and/or education may apply toward required experience. Newly graduated nurses are encouraged to apply.

PREFERRED QUALIFICATIONS:


• Graduation from a college or university accredited with a bachelor’s degree in nursing (B.S.N.)
• Associate degree in nursing (A.D.N.) AND a nursing degree beyond the B.S.N. leading to a:
  • Master’s degree in nursing (M.S.N.).
  • Nurse practitioner degree, preferably a pediatric nurse practitioner (P.N.P.), women’s health nurse practitioner (W.H.N.P.), family nurse practitioner (F.N.P.), adult health practitioner (A.N.P.), or certified nurse midwife (C.N.M.).

BILINGUAL SKILLS:

  • Applicants must demonstrate English and Spanish fluency at the time of the pre-employment screening process. Spanish language proficiency will be determined through standardized demonstration. 
    • Additional compensation will be provided based on the use of bilingual skills.

  • Working knowledge of maternal and child health or mental and behavioral health.
  • Knowledge of prenatal and early childhood services and systems.
  • Ability to effectively develop individualized health plans, counsel clients, and assess client needs.
  • Strong written and oral communication skills.
  • Knowledge of professional nursing practices.
  • Ability to interpret and apply administrative and departmental policies, rules, and regulations.
  • Ability to work well independently and be an effective team player. Must have the ability to establish and maintain effective working relationships with coworkers.
  • Ability to work respectfully with clients and families from various cultural and socioeconomic backgrounds.
  • Ability to maintain confidential data and information.
