What are the responsibilities and job description for the Connect Allen County Navigator position at Bowen Health?
Connect Allen County is committed to simplifying access to essential services and resources for youth and families through a collaborative network of onsite and offsite partner organizations. The Connect Allen County Service Navigator plays a vital role in assisting individuals seeking support by guiding them and connecting them to available resources, both within the program and across the community.
Duties/Responsibilities:
- Communicates the needs and expectations of the program to youth and families, community partners, and funders.
- Provides navigation and referral services to consumers who enter or call seeking support.
- Provides limited crisis intervention services to Connect Allen County consumers.
- Provides follow up and ongoing care, support and coaching to consumers of the program.
- Collaborates community partners.
- Maintains accurate and detailed documentation.
- Participates and supports the local system of care activities.
- Follows established strategic plans and quality improvement procedures to achieve stated goals and enhance consumer experience.
- Ensures collection of all necessary service data.
- Participates in scheduled meetings to review performance and quality indicators and makes recommended adjustments to improve operations.
- Shares knowledge from workshops and other training opportunities related to diversity, racial equity, and inclusion.
- Intervenes to explicitly address negative stereotyping and discriminatory statements or practices
when they occur. - Other duties as assigned.
Required Qualifications:
- Education: Bachelor’s Degree in Social Work or Human Services-related field.
- Certificate or Professional License: No Certificate or Professional License required
- Available to respond to all shifts.
- Valid driver’s license and active liability insurance.
- Satisfactory results from criminal and driving record background checks
->As a State-Of-the-Art Healthcare Organization and a Federal Contractor Agency – we require all individuals who join Team Bowen to receive a flu shot between the months of October - March and TB test to further protect our staff and the patients we serve. There are options for exemption applications, but approval is not always guaranteed.
