Description
The Crisis Support Specialist plays a vital role in providing support to individuals facing mental health and/or substance use challenges. In addition to assisting patients in learning and developing new skills for improved functioning and independence, the Crisis Support Specialist focuses on crisis intervention, ensuring clients are connected with healthcare providers, participating in medical appointments, and facilitating a smooth transition towards their goals. This position will help launch our mobile crisis team and will travel to and from Huntington County. This position will work 3, 12-hour shifts. 7pm-7am.

Provide immediate support during mental health and/or substance use crises, utilizing crisis intervention techniques.
Collaborate with patients to develop crisis management plans and coping strategies.
Engage in de-escalation strategies to promote a safe and supportive environment.
Assist patients in learning and developing new skills to enhance their functioning and independence.
Tailor skill-building activities to address individualized needs and goals.
Foster the development of natural support systems for ongoing client support.
Ensure clients are linked with other needed healthcare providers and coordinate their care.
Participate in medical appointments with the organization’s medical staff to provide additional support and advocacy.
Work effectively as a team member, collaborating with professionals from various disciplines to provide comprehensive care and support.
Collaborate with the multidisciplinary team to integrate healthcare goals into overall patient care plans.
Work closely with patients to identify and address the goals that are important to them and/or their referral source.
Monitor and track patient progress toward established goals through regular assessments.
Collaborate with the patient and the care team to adjust goals as needed.
Requirements:
Must be at least 21 years of age.
Must have valid driver's license and reliable vehicle.
Must meet this educational requirements:
Associates degree
Starting pay $18- $22 an hour
->As a State-Of-the-Art Healthcare Organization and a Federal Contractor Agency – we require all individuals who join Team Bowen to receive a flu shot and TB test to further protect our staff and the patients we serve. There are options for exemption applications, but approval is not always guaranteed.
