Title: Licensed Masters Social Worker
Reporting Relationship: CQO and Case Management Department Manager
General Summary of Responsibilities:
The social worker at BBGH assists families during the course of clinic visits, ER visits, and hospitalizations as the patient’s advocate and guides them through health diagnosis and illnesses. Social worker will assist families with the social, emotional, financial, and psychological concerns associated with having a loved one in the hospital. Social worker will conduct psychosocial assessments on patients and determine the need for resources. Social worker can also act as a liaison between the patient, family, health care team, and community agencies. Social worker will organize crisis intervention for patients and family members as well as provide counsel to patient and families.
The social worker’s aim is to improve the overall well-being of their patients through non-medical interventions and other types of support. The social worker’s goal is to address any social, emotional, financial, or psychosocial issues that could be impairing the patient’s recovery.
Essential Job Functions:
- Advocate for patient’s needs and concerns and communicate this to care team
- Assess each patient’s unique condition, combining data from medical diagnoses and personal rapport
- Develop comprehensive plans to help patients adjust to medical diagnoses and transition from hospitalization to normal life
- Provide necessary support for each patient according to health status, personal strengths and weaknesses, emotional state, and additional factors
- Research local resource options to determine which are most fit for a patient’s needs
- Evaluate services and resources provided to patients to determine effectiveness
- Follow up with patients throughout diagnosis, treatment, and recovery to determine the current state of social, mental, and emotional health
- Maintain current knowledge of local resources and relevant research to provide the best possible recommendations to patients and families
- Provide non-medical support to patients and their families
- Act a liaison between the patient, family, health care team, and community agencies
- Connect patients with resources and services to address social and environmental issues affecting their overall wellbeing

- Other Job Functions:

- Works closely with members of the healthcare team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
- Develops working relationships with leaders in community resource groups, representing the agency at meetings and on various committees that address the social needs of patients
- Acts as a resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems
- Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources
- Provide interventions which demonstrate knowledge of, and sensitivity toward cultural diversity and the spiritual development, and educational backgrounds of the population served
- Take creative problem-solving approaches to address complex situations
- Assisting in follow-up phone calls for all discharges, including OB, SB, surgical and behavioral health
- Identify existing or potential caregivers to expand support available to those who are living alone without a caregiver
- Participate in patient/family case conferences
- Provide emotional support for families with loved ones in a trauma or code in the ER
Job Qualifications:
Education:
Required: Masters of Social Work degree from CSWE accredited program required.
Experience:
Required: Minimum of 1-year experience in the medical field preferred
License/Certifications:
Required: Licensed Social Worker through the Department of Health and Human Services, State of NE and completed ASWB exam
