Nurse Navigator Registered Nurse Surgery Department.
POSITION SUMMARY
Provides direct patient care and comprehensive coordination of care for surgical patients and their families/caregivers, eliminating barriers to timely care, facilitates efficient flow through system navigation, high focus on patient and provider satisfaction, maintains a consistent point of contact with providers and serves as a first point of contact for patients and families requiring surgical care services. Acts as liaison between patients' staff, physicians' offices, and hospital ancillary areas.
Essential Functions
Overseas assessment, planning, implementation and evaluation of all aspects of care for patients within the scope of practice.
Works collaboratively with physicians, clinical, and department teams to facilitate, coordinate, and ensure patient access to needed surgical services and related care.
Central point of contact for all navigating patients, including interaction with medical, nursing, and ancillary services.
Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients and public.
Collects and reports data from patient on barriers, health care, medications, prior treatments, etc.
Track order completion and follow up. Monitor patient compliance with patient treatment plan.
Assist patients with referrals to outside resources and care coordination as needed.
May participate in orientation, training of new employees, travelers and students.
Follows established department policies and objectives, continuous quality improvement objectives and safety, environmental, and /or infection control standards.
Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and employee education.
Designs or participates in the creation of health education materials.
Transcribes, utilizes and implements protocols and physician orders.
Work directly with clinics to schedule patients
Confirm history and physicals are scheduled, as well as specialty doctors
Collecting history and physicals
Complete pre-op visits both in person and by phone
Communicate concerns about anesthesia and help with orders for pre-op medication
Manage pain clinic-chart prep, insurance and provider support
Assist in Pre/Post and PACU as assigned
Other duties as assigned.
Education
Required: Graduate from an approved Registered Nursing Program -- BSN preferred

Required: Nebraska RN license or multi-state Register Nurse License
BLS for healthcare workers
ACLS --
Preferred TNCC -- PALS or ENPC and NRP
