Opportunity Overview: SE NETWORK TARGETED CASE MANAGER
The mission of the Boys & Girls Club of King County is to, inspire and enable all young people, especially those who need us the most, to realize their full potential as a productive, responsible, and caring citizens. We recognize that some youth need specialized, non-traditional programming to reach their full potential. The SE Network works with community partners to provide a safety net of specialized targeted programming that works to buffer the consequences of the many risk factors that pull our youth away from Academic Success, Good Character and Citizenship and a Healthy Lifestyle.
As a Targeted Case Manager this position will work with youth referred by SE Network Intake & Referral Specialists to assess the youth’s strengths, needs and interests. This information is used to produce an Individualized Service Plan (ISP), monitor the youth’s progress, and evaluate the appropriateness and effectiveness of services.
ESSENTIAL DUTIES AND RESPONSIBILITIES: (required in person)
- consult with other relevant adults as needed to decide which provider would best serve the youth;
- assist with the connection, coordination, and collaboration with network-identified agencies, as necessary, to ensure youth is actively engaged in programs;
- assist with tracking and monitoring timelines to meet initiative reporting deadlines;
- assist in the development, tracking and reporting of outcome measurements for Network programs, activities and/or events;
- assist in document collection (field trip permission slips, membership applications, registration forms, Disclosure of Information, surveys, etc.). This includes tracking youth engagement in programs, activities and events through means such as sign-in/attendance sheets and surveys;

QUALIFICATIONS
KNOWLEDGE, SKILLS, AND ABILITIES
- Must have a High School Diploma or GED
- Bachelor Degree in Social Services or related field preferred or equivalent education and experience may be substituted for education
- At least 2 years of experience in Case management with professional trainings and/or experience providing direct multi-service to disadvantaged populations
- Experience engaging high risk youth of different backgrounds and experience in youth service field preferred
- Must have knowledge and experience working with gang-involved or -affiliated youth and families
- Ability to communicate, in writing and verbally: by writing reports and business correspondence; speaking effectively before individuals, and public groups
- Competence with Excel, Access, Word, data entry systems and computerized database management
- Ability to interview, conduct community outreach or education in community-based organization, and communicate effectively with diverse populations, including school personnel, service providers, law enforcement, and parents of referred youth, and funders
- The ability to maintain professionalism/be aware and manage personal bias and attitudes behind the scenes and in the field (Emotional Integrity)
- Current CPR and First Aid Certifications preferred
- Knowledge and/or experience working with a non-profit youth development agency, Boys & Girls Club experience preferred
- Good organization and attention to detail, professional presence, strong customer relations skills and ability to maintain strict confidentiality
- Understanding of youth development principles, ability to motivate youth in a positive way
- Ability to model and guide youth in strengths-based problem solving, goal setting and study skills
- Organized and self-motivated and able to work with others on a team and independently with little supervision
- Must have problem-solving/conflict-resolution skills
- Must be a passionate servant leader with a strong sense of calling to the field of intervention in addition to insights and understanding of the unique challenges our youth face in the SE Seattle community, these include community/school/family dynamics, risk factors and protective factors
- Ensure member confidentiality during and after employment: which includes, but not limited to membership data, and school data and other information obtained
- Adhere to agency policy, procedures and professional code of conduct
- Ability to be flexible and adapt to program changes
- Must have a valid Washington State Driver’s License
- Must have reliable independent transportation for travel
- Must be able to work some nights and weekends
HOURS: The position is full-time, 40 hours per week. Some evening and weekend work required.
LOCATION: Smilow Rainier Vista Boys & Girls Club. Some travel required.
REPORTS TO: Targeted Program Coordinator
FLSA: Hourly, Non-Exempt
SALARY: $30.00/hr, Generous benefit package including vacation, sick leave, medical, dental, vision, and bus pass stipend
