PURPOSE/FUNCTION OF PROGRAM:
The Charles F. Hayden School is committed to serving children through a psycho-educational, clinically informed therapeutic milieu. Collaborative educational planning and placement teams including families and district personnel design Individualized Educational Programs (IEP) for every student.
POSITION SUMMARY:
The School Social Worker provides clinical interventions and case management services to special education students in a milieu school setting, consistent with their IEP service plan. They act as a member of the school clinical team and provide therapeutic services to children and their families. The School Social Worker will also provide outreach to and coordinate efforts with other providers as well as maintaining routine contact with student’s families.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Completes admission, IEP Progress Summaries, PPT/IEP goals, objectives, and related assessment and discharge summaries.
- Provides clinical services as dictated by the IEP service plan including group and individual treatment, family contacts, collaboration, case management, and home visits when appropriate.
- Completes documentation for all services provided in the electronic record as well as any hard copies the home school district may require.
- Develop Crisis Behavior Assessment and ensure proper implementation in the milieu
- Meets regularly for supervision with assigned supervisor.
- Attend Kid’s INN/Safe Haven/School rounds as assigned.
- Attends and facilitates (as assigned) team meetings and group/psychiatric supervision meetings as scheduled.
- Maintains a flexible work schedule in order to provide needed services to children and families
- Adheres to and supports the milieu behavioral/social skills model as well as Therapeutic Crisis Intervention guidelines.
- Gathers QI measures and prepares for certification site visits if needed.
- Interfaces with parents, outside providers, etc to provide comprehensive services for students.
- Participates in PPT meetings with required information regarding specific cases, relevant issues and related resources.
- Collaborates with classroom teacher regarding in-class data acquisition in direct relation to all social and behavioral objectives and prepares a report of updated progress to present at the PPT at least 5 days prior to the meeting (documents should be sent to the school district and guardian).
- Administer and score behavioral and social benchmark assessments/ data three times per year (using Aimsweb).
- Accepts other assignments and responsibilities as assigned
ESSENTIAL QUALIFICATIONS/SKILLS:
- Master’s Degree in Social Work or Master’s level degree in related field
- Minimum of two years’ experience working with children and families in a clinical setting
- Must be certified by the State Department of Education as a School Social Worker, Counselor or Psychologist
- Must fulfill competency based training requirements based on agency and program needs
- Must be able to respond to crisis, including the ability to physically restrain if necessary
PREFERRED QUALIFICATIONS:
- Three to five years previous experience working with children with emotional and behavioral challenges and their families in a therapeutic or educational setting.
- Experience in developing and running groups in a milieu setting
- Bi-lingual (Spanish) Desired
SUPERVISORY RESPONSIBILITIES:

PHYSICAL DEMANDS:
- Must be in sufficient physical condition in order to engage in a Therapeutic Crisis Intervention
- Ability to implement Charles F. Hayden School interventions and services
- Must complete CPR and First Aid training
WORKING CONDITIONS AND ENVIRONMENT:
- Ability to maintain a flexible work schedule
- Must have a valid driver’s license and be willing to drive off campus for home visits and/or collaborative meetings
