PRIMARY FUNCTION:
This job description is intended to convey information essential to understanding the scope of a Level 1 Case Manager/ mentor. Under direct supervision, of the Director of Community Health and Wellness you will receive training to regularly meet with youth participants ages 10-18, helping each create individual goals and track their accomplishments, with the end goal of preventing youth involvement with violence through mentoring under OJP.
The OJP Case Manager/ Mentor provides mentoring services to youth ages 10-18 years of through our National Mentoring Programs funding by the Department of Justice (DOJ), Office of Justice Programs (OJP), Office of Juvenile Justice and Delinquency Prevention (OJJDP). The funding is being provided to promote positive outcomes and reduce risk factors for at-risk and high-risk youth in diverse communities throughout the country. All mentoring activities and interactions should be site-based to take place at the funded site.
KEY ROLES (Essential Job Responsibilities):
- Always practice and lead with kindness, respect and compassion
- Provides crisis intervention as needed and or resources.
- Documents all services provide in case file.
- Adheres to compliance with any grant contract requirements.
- Demonstrates an ability to communicate critical concerns and treatment needs.
- Case manager mentor will provide personalized services based on mentoring assessment needs.
- Use BGCA forms and guidelines/ complete mandated BGCA trainings
- Implement BGCA prevention and/or intervention component which may include: BGC based programming, academic support, weekly case management services, workforce development, mentoring and drug/alcohol prevention programming.
- Provide information and referral assistance, administer surveys, collect data, and coordinate services with Boys & Girls Club and referral agency.
- Performs other duties as assigned
POSITION REQUIREMENTS (Responsibilities):
1. BA or AA and/ or previous case management experience of 4yrs
2. Bilingual skills preferred.
3. Meets Federal Bureau of Investigation and Department of Justice clearances.
4. Possess Class C California Driver's License and insurable driving record.

RELATIONSHIPS:
Internal: Maintain close, daily contact with Club staff (professional and volunteer), Club members, and supervisor to receive/provide information, discuss issues, explain or interpret guidelines/instructions; instruct, and advise/counsel.
External: Maintain contact as needed with external community groups, schools, member's parents and other to assist in resolving problems.
