Description
Responsible for providing case management services. Work schedule will reflect meeting the requirements under the essential job functions and will be overseen by the Case Management Coordinator. All employees of Boysville, Inc. receive training in the Quality Improvement (QI) activities of the company and are required to actively participate in QI meetings, tasks, projects and/or committees, as needed.
Essential Job Functions:
- Provide direct casework services to all assigned children from the age of infancy to 17
- Monitor and coordinate the delivery of services to children from intake to aftercare
- Complete initial social histories
- Complete regular six-month Plans of Service involving child assessments
- Complete Quarterly Reviews
- Monthly monitoring of off campus foster homes
- Perform regular home visits in foster homes to conduct assessments of childrens progress
- Document and maintain childs data on a monthly basis
- Maintain a complete computer and paper file on each child in case load
- Make relevant referrals
- Coordinate and facilitate timely implementation of discharge plans
- Maintain continuing communication with children, foster parents, parents, TDFPS caseworkers, attorneys, CASA volunteers and others involved in case planning
- Respond to emergencies when needed
- Act as a liaison between Boysville and TDFPS
- Attend Interdisciplinary Team Meetings and Assessment Team Meetings
- Attend Administrative Staff Meetings
- Serve on PQI Records Review Committee
Qualifications
- Must have a Bachelors Degree in Social Work or related Human Services field
- Licensed Bachelor Social Worker and 1 to 2 years experience in a similar position, preferred
- Must possess excellent communication, organization, time management and written skills
- Must be computer literate in windows applications
- Must exhibit behavior that demonstrates cultural diversity and sensitivity (See Corporate Policy #2103), which includes sensitivity to our service populations cultural and socioeconomic characteristics
Physical/Mental Requirements:
- Ability to read, write and converse in English
- Ability to communicate with staff, residents, families, vendors, support agencies and others
- Ability to handle stress
- Bending, lifting, grasping, fine hand/eye coordination, pushing/pulling, prolonged sitting/standing
- Ability to respond to telephones and other auditory stimulation. Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements)
