- Support the social service function in a long-term care and short-term rehabilitation nursing center.
- Work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related to psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans.
Qualifications:
- High School Diploma or equivalent required.
- Post-secondary education or work experience in a social service or health related field a plus (such as social services, social work, patient registration, care coordination, or discharge planning).
- Must be familiar with Medical Terminology.
- Must be proficient in the use of personal computers (Windows, Internet, and Applications/Programs navigation)

ENTRY QUALIFICATIONS
Minimum high school diploma or equivalent required.
One or more years experience in health care education/ marketing / insurance or commensurate educations preferred.
Must be familiar with medical terminology.
Proficient in the use of personal computer.
