What are the responsibilities and job description for the Intake & Documentation Specialist (LPN) position at Bradley County Medical Center?
The Home Health Intake & Documentation Specialist will have an active, in good standing AR LPN license, with 3 years of experience in clinical documentation. Home Health experience is preferred. This position reports directly to the Director of Home Health, and will operate out of the Brunson Medical Complex.
Primary Responsibilities:
Intake & Coordination:
- Collects and verifies accurate referral information
- Completes initial patient intake forms (admission records).
- Verifies insurance benefits for private payers and completes the insurance verification process.
- Obtains and records physician orders for home health services, ensuring clear and accurate documentation.
- Communicates physician orders to the home health agency (HHA) care staff efficiently and accurately.
- Demonstrates knowledge of the agency's admitting criteria and regulatory guidelines.
- Ensures compliance with state scope of practice, accrediting body standards, and federal and state home health regulations.
OASIS & Clinical Documentation:
- Assists in the review of clinical information for accuracy and completeness as it relates to OASIS data and ICD-10 coding, ensuring compliance with the Medicare PDGM billing model and CMS guidelines.
- Identifies inconsistencies or missing documentation and communicates recommendations to clinicians to promote OASIS accuracy.
- Flags potential corrections or documentation needs for review by the RN or clinical leadership.
- Ensures timely preliminary review of OASIS documents, maintaining productivity expectations set by the Director.
- Assists in the export and validation process of OASIS data submission, ensuring timely and accurate data entry.
- Supports efforts to reduce Medicare denials by ensuring that coding and documentation meet initial compliance standards.
- Collaborates with RNs, therapists and other disciplines to support smooth documentation workflows.
Plan of Care Assistance:
- Reviews Plans of Care (POC) for completeness and consistency before submission for RN approval.
- Ensures POC documentation includes appropriate goals, interventions, and face-to-face encounter details, referring discrepancies to an RN or supervisor.
- Supports the interdisciplinary team in aligning care plans with patient needs and physician directives.
Additional Duties:
- Provides administrative and clinical support to the home health team as needed.
- Communicates effectively with nurses, therapists, physicians, and administrative staff to ensure seamless care coordination.
- Participates in ongoing OASIS and ICD-10 training as required.
- Performs related duties as assigned by the RN, Manager, or Director within the LPN scope of practice.
