LPN
A beautiful 30 bed, Geriatric Skilled Nursing Facility with an attached 74-bed residential care home seeks Part time LPN. The Bradley Home has been part of the Meriden community since 1937. The home is a continuum of care and has consistently earned Medicare Five Star Recognition. Outstanding resident care is our mission and our facility has an overall low resident acuity.

Duties:
1. Assumes the responsibility and accountability for completing assigned tasks and duties in a timely manner. Performs all duties in accordance with regulations, policies, procedures, and rules of safety. Assures follow-through and continuity of care and achieves superior outcomes.
2. Assures a clean, safe, healthy living and working environment, reporting all unclean, unsafe, hazardous conditions and equipment to the appropriate supervisor.
3. Assures that established infection control policies and procedures are followed and is able to effectively institute measures or procedures to control the spread of infection/communicable disease.
4. Assures and protects residents’ rights to fair and equitable treatment, self determination, individuality, dignity, privacy, confidentiality, property, and civil rights, including the right to wage a complaint are followed according to the standards set by the Bradley Home.
5. Maintains confidentiality of all resident information and assures that all information in the residents’ medical record is protected.
6. Assures that each resident enjoys total freedom from abuse or neglect and reports suspected or obvious abuse or neglect to appropriate supervisory personnel, according to policy.
7. Makes careful decisions within scope of practice regarding resident care and seeks appropriate input from the Shift RN Supervisor.
8. Evaluates emergencies and determines emergency measures to be used along with the Shift RN Supervisor. Does not make an initial assessment of a resident who has a medical or life threatening emergency, a significant change of condition, incurred an injury from a fall, or any other reason. Does not record the information from the RN’s initial assessment on the medical record.
9. Monitors, evaluates, and reports resident symptoms, conditions, problems, needs, and functional status to Shift RN Supervisor.
10. Provides professional nursing services according to regulations such as obtaining lab specimens, performing prescribed treatments, vital signs, emergency care and treatment, special therapies, etc., according to policies and procedures.
Education:
· Graduate of an approved LPN program.
License
