In this role, you’ll help create and deliver Cognitive Care Plans to patients living with dementia, Alzheimer’s, and other cognitive disorders. You’ll work closely with Dr. Hemant Pandey to support patients and their caregivers in both Casa Grande and Chandler.

- Providing educational resources and support
- Documenting visits accurately
- Supporting the neurologist during assessments
- Building lasting relationships with patients and caregivers
