Provides general care and education to patients under the direction of a RN or BSN. Performs tasks including scheduling, record-keeping and maintaining supply inventory
Essential Job Functions:
- Takes patient vital signs
- Performs common tasks including but not limited to:
- Treating bedsores
- Dressing wounds
- Bathing
- Dressing
- Coordinates visits and paperwork for patient/doctor visits.
- Record client’s progress and conditions
- Records and reports symptoms.
- Reviews patient charts and records essential information including medical past and history, reactions, treatments, etc.
- Assists patients with daily hygiene needs.
- Maintains inventory and stock of clinical and medicinal supplies.
- Educates patients on current health issues.
- Practices safety and health code standards.
- Performs jobs assigned by head nurses and doctors as directed.
Job Qualifications:
Educations: Degree as a Licensed Vocational/ Practical Nurse
Licensure: Current Texas State License as an LVN/LPN, current Texas Driver’s License.
Experience: 6 months experience in acute care or rehabilitation setting, home health preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation and valid and current auto liability insurance.
Environmental and Working Conditions
Works in a patient’s home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather.
Physical and Mental Efforts:
Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs.
