Responsibilities

- Support clients in following their care plans and maintaining their independence.
- Monitor and document changes in clients' health or behavior and report any concerns to the appropriate healthcare professionals.
- Provide companionship and engage clients in social activities to promote emotional well-being.
- Assist with mobility and transfers, including heavy lifting when necessary.
- Ensure client safety by adhering to resident rights and maintaining a clean environment.
- Collaborate with family members, healthcare providers, and social workers to ensure comprehensive care.
- Provide hospice care as needed, ensuring comfort during end-of-life stages.
Skills
- Knowledge of resident rights and the ability to advocate for clients' needs.
- Experience in assisted living or home care settings is preferred.
- Familiarity with developing and implementing care plans tailored to individual needs.
- Understanding of hospice care principles and practices.
