Job Summary:
We are seeking a compassionate, patient, and reliable Community Integration Specialist/Personal Assistant to support teen with an intellectual/developmental disability in engaging with community activities and fostering independence. The ideal candidate will assist with social outings, daily activities, and skill-building, ensuring a safe, supportive, and enriching experience that promotes inclusion and personal growth. Consistency and adherence to schedule are essential to ensure stability and routine.
Key Responsibilities:
Community Engagement:
- Assist in identifying and facilitating participation in community events, recreational activities, and volunteer opportunities.
- Encourage social interactions and relationship-building with peers.
- Provide transportation to and from community events, classes, and appointments.
Personal Assistance:
- Support with daily living activities such as meal planning, light housekeeping, and organization.
- Encourage and promote independence in routine tasks.
Skill Development:
- Support the individual in learning and developing life skills, such as budgeting, time management, and communication.
- Provide guidance in using public transportation, navigating public spaces, and engaging in meaningful activities.
Advocacy and Support:
- Act as an advocate to ensure the individual’s voice is heard and respected in the community.
- Encourage self-advocacy and informed decision-making.
- Offer emotional support and encouragement.
Safety and Well-Being:
- Ensure a safe environment in all activities and outings.
- Recognize and address any challenges or barriers to participation in community life.
- Monitor health and well-being, reporting any concerns to family or healthcare providers.
Communication and Documentation:
- Maintain clear and open communication with family members, caregivers, and community organizations.
- Document activities, progress, and any incidents as required.
- Collaborate with other support professionals and service providers.
Qualifications and Skills:
- Experience working with individuals with disabilities is preferred.
- Strong interpersonal and communication skills.
- Patience, empathy, and a positive, encouraging attitude.
- Ability to work independently and adapt to changing needs and environments.
- Consistency and adherence to schedule are critical to success in this role.
- Valid driver’s license and reliable transportation required.

Working Conditions:
