Care Coordinator with Home Care Experience
Brazos Home Care is seeking a professional and experienced care coordinator and scheduler to accommodate scheduling, caregiver communication, and client communications. Good customer service and bedside manner is a must! Do you have excellent communication skills, ability to work under pressure, and a caring and compassionate personality? We are looking for our next Brazos Home Care team member! Must have reliable transportation that is not shared. Ability to be in office from 8am-5pm. Strong organizational and time management skills. Ability to effectively communicate positively with clients and caregivers. MUST be highly organized and motivated.
Duties include:
Assigning and coordinating staff to client schedules.
Handling all incoming calls.
Assisting with after-hours communications.
Effective and immediate documentation of all communications.
Ability to fill in shifts when necessary.
Communicating with field staff and clients via phone.
Ability to answer on-call as scheduled without interference after hours.
Home care scheduling is required.
Reliable (personal) transportation is required.
