Autism Breakthrough provides a range of tailored services to adults with autism and their families and serves as a resource for the broader community. This includes creating a community-based housing that serves adults with a diverse range of abilities. Our goal is to teach, encourage, and support individuals with disabilities to live rewarding lives.

The Transition Specialist exists to ensure all people supported have the services and supports needed to have a meaningful day and to live their vision of valued community living. This position is responsible for onsite oversight of adults with autism in our independent living program at the Anchor House. The Transition Specialist acts as a support for the individuals in the program and helps teach each individual essential life skills to graduate from the program within 2 years. All duties are performed with the knowledge that Breakthrough strives to be the model of excellence for adults with autism.

Requirements

A. General Responsibilities


1. Implement Community Integration standards of excellence in the assigned homes. 

2. Supervise, train and develop staff who work with the persons supported living in the homes they are assigned. 

3. Serves as a point of contact for staff in the home for communication with program managers/director via daily site visits.

4. Daily Site visits will be used to ensure safety and cleanliness of the homes and documented on site visit checklists.

5. Collect staff and person supported documentation in a timely manner (Missed Punch, Mileage, Appointment forms, Accident/incident reports, etc.). 

6. Ensure all daily communication logs (med counts, money folders, etc) are completed in a timely manner

7. Alert program managers/director about issues with staff on quality of documentation via daily note reviews (daily notes, sleep logs, intake and elimination logs etc). 

8. In coordination with Community Integration Scheduler (CIS), ensure proper staffing ratio is implemented based on the master schedule and notify CIS and program managers about call offs/no shows as needed keeping a weekly calendar and call off log that will be turned in to CIC at end of the week. 

9.Helps program managers/director arrange on-call coverage in emergencies, including temporarily required to help provide coverage while emergency is resolved.      

10. Conducts drills/site assessments and report according to Breakthrough and /or the State of Tennessee, Department of Intellectual and Developmental Disabilities (DIDD) procedures. 

11.Notifies the Health and Individual Rights Coordinator (HIRC) and program managers/coordinator of all emergency situations to provide for the safety of the persons supported. If necessary, contact appropriate emergency services to ensure the safety of the supported individuals. 

12. Monitors the facility for safety, cleanliness, and needed repairs. Address concerns or report to appropriate party. 

13. Assists Case Managers in evaluating and implementing persons supported activities of daily living, meeting the goals stated in the Individual Support Plan (ISP) or Person Centered Support Plan (PCSP) to provide a meaningful day. 

14. Assist with evaluating the technological needs of each person supported and reporting that to case management for review. 

15. Facilitate attendance and follow up of all medical appointments for appointed homes/persons supported. 

16. Communicates with the Health and Individual Rights Coordinator and Health case manager regarding medication, as needed, for each person supported and ensures delivery of the required documentation to the administrative offices. Assists in delivery of medications to homes as requested. This includes notifications to assure all requests for refills are notified to the HIRC in a timely manner, medication closets are properly maintained, Leave of absence meds are prepared and certified med-safes are on site as needed. 

17. Assists in the assurance of completion of General Event Reports and assists in collecting evidential photos as needed. 

18. Helps track health data and report it to the Health and Individual Rights coordinator and Health Case Manager as needed. 

19. Ensures that the nutritional guidelines for each person supported are followed as written and available in home to staff during mealtimes. This includes menu planning to ensure staff have appropriate meals for the person supported. 

20. Assists with shopping for persons supported in assigned homes (including but not limited to: groceries, household supplies, personal items, etc.) and helps staff and the Person supported go on the shopping trip (when appropriate) to help facilitate their choice in items. 

21. Reports to program manager about issues that arise with staff/persons supported. 

22. Abides by all policies, procedures, regulations and guidelines for safety, quality assurance and general corporate oversight of Breakthrough and/or DIDD.

23. Facilitate teamwork between all team members via open communication, sharing of responsibilities and a positive, constructive attitude.

24. Carry out all responsibilities professionally and efficiently.

25. Work in harmony with all agency staff as well as staff of outside entities.

26. Provide role quality improvement recommendations to the community manager. 

27. Assists in the performance of other duties required by the needs of Breakthrough and/or the persons supported and as assigned by the Executive Director, Director of Operations, or Residential Director.
