Job Description
Are you an experienced MDS Coordinator passionate about delivering exceptional care in a skilled nursing facility? Breezy Hills Rehabilitation and Care Center is seeking a motivated Registered Nurse (RN) to join our team full-time!
Key Responsibilities
- Ensure Quality Care : Oversee care standards to guarantee our long-term residents receive the highest level of support.
- Conduct Assessments : Perform comprehensive assessments and interviews for new residents to create individualized care plans.
- Lead Care Planning : Develop personalized care plans with measurable goals for short-term and long-term improvement.
- Collaborate Across Teams : Host interdisciplinary meetings with healthcare professionals, families, and caregivers to ensure holistic care approaches.
- Community Engagement : Partner with specialists to achieve optimal resident outcomes.
- Provide Leadership & Education : Conduct nursing management meetings and deliver in-service training on MDS, RAPS, and Triggers to ensure staff are informed and proactive.
- Maintain Compliance : Manage the MDS / Care Planning schedule with a focus on non-Medicare residents.
