Summary of Role:
As the Community Navigator at Brethren Hillcrest Homes, you will play an essential role in forming relationships with new residents and assisting them as they prepare to move to Hillcrest.
The Community Navigator's Responsibilities:
- Assist residents in planning the logistics of their move.
- Orient residents to their new home.
- Introduce residents at various events.
- Provide a comprehensive New Resident Orientation to Hillcrest that addresses: Hillcrest campus and facilities, departmental services, levels of living and care (Continuum), Dining Services and venues, life enrichment opportunities, the Resident Handbook and Telephone Directory, Residents Association Committees. rmation about community resources and services.
- Regular check-ins during their first 90 days and updates relevant parties.
- Facilitate workshops and informational sessions on relevant topics for residents.
- The Community Navigator is a member of and actively participates on the Care Coordination Team.
- Will attend weekly meetings to provide relevant updates on new residents as well as those needing assistance or preparing to transition between levels.
- Communicate with associates in all departments to understand and help address the changing needs of residents.
- Educate residents about Hillcrest’s services and levels of living.
- Follow transfer protocols as specified in regulation.
- Coordinate the logistics of resident transfers.
- Work with the Sales and Remodeling/Refurbishing teams to coordinate releases of homes and apartments and prioritize completion of remodels to accommodate internal transitions.
- Maintains a list of residents who have expressed an interest in voluntarily moving to another home/level of living within the community.
- Manage both voluntary and involuntary transitions as requested by residents or as prioritized by the Care Coordination Team. This includes all logistics (i.e., projected timelines, moving services, family involvement, etc.)
- Works with residents, families, and staff to orchestrate internal moves smoothly, compassionately, and in a timely manner.
- Work closely with Hillcrest’s clinical and social services teams to understand the needs of residents to help coordinate supportive services.
- Monitor and maintain occupancy levels and ratios at the targeted levels.
- Anticipate resident moves and proactively engages them in conversations and visits to minimize transfer trauma.
- Ethically manage resident transfers to meet resident needs within the operational framework of Hillcrest.
- Understands the importance of sustaining a stable occupancy for all residents.
- Proactively represents Hillcrest services and all levels of living to residents, family members, guests, and associates.
- Participate in community events both on-and off-campus.
- Any other responsibilities that may arise in facilitating a resident’s understanding of benefits associated with a transition and the smoothest experience possible.
Qualifications
- Warm, compassionate, personality with the ability to empathize with others while balancing resident personal needs and the Hillcrest continuum of care business model.
- Experience with electronic clinic systems is preferred.
- Excellent project management capabilities to oversee multiple initiatives effectively.
- Exceptional public speaking skills to engage residents and stakeholders effectively.
- Social work, nursing, or background in a CCRC setting is preferred.
- Ability to work collaboratively and effectively with diverse groups of people.
- Currently holds or able to obtain RCFE license within 90 days of hire.
