A Magnet-designated hospital in Southern Vermont is searching for a Registered Nurse Case Manager to join its growing team! In this role, you will utilize the case management process to assess patient needs, develop treatment plans, schedule appointments, and act as a liaison between patients, providers, and various third parties. Responsibilities:
- Assessing patients, gathering information, reviewing diagnoses, and analyzing medical test results.
- Developing care plans that meet the needs of patients and revising plans based on patient’s progress toward goals.
- Educating patients and families about care plans and their options.
- Scheduling appointments for doctor visits and other medical services as needed.
- Updating patient’s medical records.
Qualifications:
- A BSN is required.
- Active RN License.
- 3 years of nursing experience is required.

- Schedule: Monday-Friday, 8a-430p
