- As a case manager (RN), you will be responsible for the managing resident cases.
- You will evaluate resident's medical history, needs, and support network.
- You will chart and maintain documentation for residents.
- You will design resident treatment plans.
- You will develop and maintain a positive working rapport with all team members.
Case Manager Qualifications:
- Bachelor's or Master's Degree in Human Services
- Current CPR certification.
- At least three years experience in long term care.
- SMHRF experience, preferred.
