The Wound Care Nurse Practitioner or Physician Assistant is vital in providing specialized care for patients with acute and chronic wounds, including pressure ulcers, diabetic foot ulcers, venous ulcers, and post-surgical wounds. This NP will collaborate with a multidisciplinary team to develop and implement customized care plans, conduct thorough wound assessments, and focus on achieving optimal patient outcomes.

Wound Care Nurse Practitioner Duties:
- Conduct comprehensive patient assessments.
- Develop, implement, and evaluate individualized treatment plans for residents
- Order and interpret diagnostic tests related to wound care.
- Provide specialized care for patients with acute or chronic wounds.
Wound Care Nurse Practitioner Requirements:
- Master's or Doctorate in Nursing from an accredited program
- Current Illinois Nurse Practitioner license
- Wound Care certified, preferred
- Strong clinical assessment and decision making skills
- Experience in acute care or skilled nursing
