Key Responsibilities :

Manage and order prescribed medications, supplies, and equipment

Implement individualized care plans based on resident needs

Perform wound care, treatments, and procedures with attention to detail

Accurately document resident care, including observations, interventions, and evaluations

Protect resident confidentiality and comply with HIPAA regulations

What We’re Looking For :

Valid LPN license in the State of New York

Experience in skilled nursing preferred
