Position: Social Work Assistant
Shift: Day Shift (9 AM - 5 PM or 8 AM - 4 PM)
Pay: $18 - $23 per hour

Position Overview:
As a Social Work Assistant you will play a crucial role in supporting the Director of Social Services by efficiently managing clerical duties and providing assistance to residents and their families. This position requires a strong foundation in customer service and excellent communication skills to interact effectively with residents, families, and the interdisciplinary team.
Key Responsibilities:
- Maintain accurate, updated, and organized records within the department.
- Assist residents and families in obtaining benefits from local community and government agencies.
- Conduct admission assessments for new residents.
- Update and review the weekly discharge calendar.
- Update and review weekly Utilization Review (UR) tracking documents.
- Send out care plan invitations to residents and families.
- Assist in organizing care plan meetings, ADHOC meetings, and discharge planning meetings.
- Promptly respond to and follow up on daily emails and phone calls.
- Provide comprehensive assistance to residents and families with all aspects of care.
- Answer incoming calls, respond to emails, and follow up on pending matters as delegated by the Director of Social Services.
Qualifications:
- Must be currently enrolled in college pursuing a degree in Social Work or a similar major.
- Proficiency in Microsoft Office and Microsoft Teams.
- Excellent customer service skills.
- Strong data management and organizational skills.
