Key Responsibilities :
Ensure that effective, quality nursing care is delivered with a focus on outcomes by utilizing the nursing process. Collaborate on discharge planning throughout the resident's stay.
Develop individualized care plans in partnership with the resident and the interdisciplinary care team.
Implement care plans efficiently, focusing on resident-centered outcomes while utilizing available resources for counseling and discharge planning.
Deliver and uphold the highest standards of resident care by applying knowledge of current nursing practices.
Qualifications :
Current license to practice as a Licensed Practical Nurse (LPN) in the state of practice
Current CPR certification is required; nursing staff must obtain CPR certification within 90 days of hire
Recent clinical experience and / or education in geriatrics
