    • Key Responsibilities :

  • Complete the Resident Assessment Instrument according to federal / state regulations and company policies.
  • Serve as an in-house case manager, coordinating care with physicians, families, third-party payers, and facility staff.
  • Further details regarding responsibilities and physical requirements will be provided during the interview process upon request.

    • Qualifications :

  • Licensed Practical Nurse or Registered Nurse with a current, active license in the state of practice.
  • Current CPR certification required.
  • A minimum of two (2) years of clinical experience in a healthcare setting.
  • At least one (1) year of experience in long-term care.
  • Previous experience as an MDS coordinator with proven expertise.
  • Ability to maintain consistent attendance.
