Job Description
The MDS Coordinator is responsible for overseeing patient assessments, ensuring compliance with federal and state regulations. This role involves coordination of resident care with various stakeholders, including physicians and families.
Responsibilities
- Act as an in-house case manager by coordinating services with physicians, families, third-party payers, and facility staff.
- Conduct thorough assessments to identify resident needs and develop effective care plans.
Requirements
- Licensed Practical Nurse or Registered Nurse with a current, active license in the state of practice.
- Maintain a current CPR certification.
- Minimum two (2) years of clinical experience in a healthcare setting.
- Minimum of one (1) year of experience in a long-term care setting.
- Prior experience as an MDS coordinator with demonstrated expertise.
